 MODERATED e-POSTER SESSION 06:
Infectious Diseases and COVID-19
PP41
Objective: The aim of our study was to describe the incidence of infectious complications of endobronchial ultrasound guided transbronchial needle aspiration (EBUS-TBNA) and to analyze the potential risk factors in a prospective cohort of patients.
Methods: We conducted a prospective multicentre study, with all consecutive patients referred for an EBUS-TBNA with patients at risk of developing an infectious complication (considering >10 nodal samplings, known immunosuppression, bronchial colonization and necrotic lesions).
Results: One hundred ninety were included. Overall, 10 patients (5.2%) presented an acute infectious complication: 4 patients presented pneumonia, 1 mediastinitis and 5 mild complications (respiratory tract infection
that resolved with antibiotic). One-month follow-up
showed 1 mediastinitis at sixteenth day post-EBUS, which required surgical treatment, and 1 pneumonia and 1 respiratory tract infection at nineteenth day. All patients had a good evolution and there were no deaths related with infectious complication. No association was found between potential risk factors and the presence of complications.
Conclusion: The incidence of infectious complications in a subgroup of patients at risk in routine was higher than previously described, but remains low, without clear risk factors. No fatal complication occurred, so reinforces the idea that EBUS-TBNA is a safe technique for assessment of the mediastinum.
PP42
Introduction: During the COVID-19 era, an increased need for ICU admission, long-term intubation and high rate of tracheostomy have been associated with an increase in airway complications requiring interventional pulmonology management.
Methods: All patients referred from the COVID ICU of our tertiary University Hospital, for IP management during the last 12 months, were recorded and analyzed.
Results / Conclusion: 10 calls for urgent airway management in patients hospitalized for COVID in our ICU, were recorded: 5 cases of Post-Intubation subglottic tracheal Stenosis, 2 cases of tracheoesophageal Fistula, 1 case of foreign body (nasopharyngeal cannula) aspiration, 1 case of pneumothorax and severe air leak from the lung periphery and 1 case of severe bleeding occluding the bronchi with clots were all managed with various bronchoscopic modalities. Given the increased ICU admissions, post intubated patients’ complications could be increased respectively. It is essential to maintain a high level of suspicion for these complications and include them in the differential diagnoses of airway disorders observed in patients either during hospitalization or after discharge. Rare diseases, such as post-intubation tracheal stenosis, might be much more common in the following years.
